Position Summary:
Compassionate Home Health LTD is seeking a motivated and compassionate Per Diem Home Health Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes throughout the Greater Sacramento Area.
The Home Health LVN is responsible for providing skilled nursing services to assigned patients under the direction and supervision of the Registered Nurse Case Manager and in accordance with physician orders, individualized plans of care, and agency policies and procedures.
The LVN provides compassionate, patient-centered care including skilled nursing visits, medication management, wound care, patient education, and monitoring of patient conditions. The LVN collaborates with the interdisciplinary team to promote positive patient outcomes and ensure care is delivered in compliance with accrediting organization standards, Federal and State regulations, reimbursement guidelines, and agency requirements.
Participates in Quality Assessment and Performance Improvement (QAPI) activities.
Duties:
- Providing skilled nursing visits to patients in their homes according to the established plan of care
- Performing patient assessments and reporting changes in patient condition to the RN Case Manager and physician as appropriate
- Providing wound assessments, wound care treatments, and monitoring patient progress according to physician orders and established protocols
- Assisting with the management of patients with complex medical conditions and chronic illnesses
- Monitoring vital signs, medications, symptoms, and overall patient progress
- Performing treatments and procedures within the LVN scope of practice
- Reinforcing patient and caregiver education related to medications, treatments, disease management, safety, and preventative care
- Collecting and documenting accurate clinical information in the electronic medical record (EMR)
- Completing timely and accurate visit documentation, including skilled nursing notes and required clinical updates
- Communicating effectively with patients, families, physicians, RN Case Managers, and the interdisciplinary team
- Collaborating with the RN Case Manager regarding changes in patient status, care needs, and patient progress
- Following infection prevention, safety, and quality standards
- Participating in agency training, education, and Quality Assessment and Performance Improvement (QAPI) activities
Regulatory Requirements:
- Current Licensed Vocational Nurse (LVN) license in the State of California
- Current BLS/CPR certification
- New graduates are welcome to apply! We provide training and mentorship for motivated LVNs interested in developing their skills in home health nursing.
- Minimum one (1) year of clinical nursing experience preferred; however, new graduates with strong clinical skills and a willingness to learn will be considered
- Prior home health experience preferred but not required
- Wound care experience preferred; training and support available for LVNs interested in expanding their wound care skills
- Experience with skilled nursing visits, medication management, patient education, catheter care, and chronic disease management preferred
- Valid California Driver’s License, current automobile insurance, and reliable transportation
Skills:
- Strong clinical assessment and communication skills
- Ability to work independently while collaborating with the care team
- Knowledge of current Federal, State, accreditation, and agency requirements related to home healthcare
- Computer skills and ability to document using electronic medical record (EMR) systems
- Strong organization, time management, and documentation skills
- Ability to provide compassionate care in a patient’s home environment
Schedule:
- Per Diem position with flexible scheduling
- Availability based on patient needs and assigned visits
- Ability to travel throughout the Greater Sacramento Area
Compensation: $35.00 - $45.00 per hour

Our CFO/COO/Founder is Dr. Louie Oberman. After graduating with a degree in Physical Therapy in the Philippines, she came to the United States in 1990. In 2010, she was awarded a Doctorate of Physical Therapy from Utica College Syracuse. She has over 20 years experience in different clinical skills settings, specializing in Geriatric Care (Certified in Wound Care, Kinesiotaping Geriatric, ACP Patterned Electrical Neuromuscular Stimulation; Certified Instructor; Craniosacral and Myofascial). She has several years of clinical experience in handling complicated and non-complicated Ortho cases; Orthotics and Prosthetics, Neurological and Cardiovascular cases, Dementia and other Central Nervous System Disorders. She has also developed Rehab Department programs from the ground up as a consultant in managing multi-site facilities (in CA, CO, KS, MO) in conjunction with educating peers and students by enhancing their clinical skills. She has become an expert in handling complex cases.
We have two registered nurses, Manuelito “Maui” Aruta, RN, DPCS/Administrator/Co-Founder and Charles Young, RN, DPCS Designee/Co-Founder, on our team. They both have over 20 years of experience in Nursing, specializing in Peri-Operative and Intensive Care Unit (ICU) and Medical Surgical (ICU), and in Neuroscience with a remarkable management background.
Last but not least is Maricar Venegas, CEO/President/Co-Founder. Maricar owns multiple Board and Care Facilities in Sacramento. She is well versed and profoundly experienced working in a patient/clients service based environment, helping patients to get better and to live with an astounding quality of life.
